Acknowledgement of Risk

I understand that participation in activities at Gundels Fun Zone (including but not limited to VR games, VR Escape Rooms, Soccer Pool/Snook Ball, inflatables, climbing structures, and open play areas) involves inherent physical risks, including but not limited to injury, illness, stress reactions, and equipment failure.

I understand that VR experiences may include rapid movement, flashing lights, immersive content, and emotional stimulation, and may not be suitable for all participants.

Age & Supervision Requirements

• VR Escape Room games with scary content require participants to be 18+, or include an adult participant.
• Groups with children under age 12 in any Escape Room must have an adult actively participating or assisting.
• VR Arcade games are suitable for all ages; however, participants under 18 must have an adult present in the facility for supervision.
• Children must have reasonable awareness of their space and an understanding of the content (example: not attempting to jump in VR water, run/climb on furniture or structures due to stress/excitement).

Facility Rules

• No gum or food is permitted during play.
• Participants must follow staff instructions at all times.
• Running, climbing on furniture or structures, or unsafe behavior is strictly prohibited.
• VR equipment must be handled with care; intentional damage will result in replacement charges.

Medical Consent

In the event of injury, I authorize Gundels Fun Zone staff to provide basic first aid and/or seek emergency medical treatment on behalf of myself and/or the minor child(ren) listed above.
I accept financial responsibility for any resulting medical expenses.

Photo/Video Release

I grant Gundels Fun Zone the right to use photographs or video taken of myself and/or the minor child(ren) listed above during activities for promotional purposes without compensation.

Liability Waiver

I hereby release and discharge Gundels Fun Zone, its owners, staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising from my participation or my child(ren)’s participation in activities at the facility, except in cases of gross negligence or intentional misconduct.

I acknowledge that by signing this document, I am waiving certain legal rights, including the right to sue, and that I am signing this waiver freely and voluntarily.

I acknowledge that I have read and understand this waiver in its entirety and that I am giving up some rights of due process by signing this document.
